Social Services

Our social workers assist residents and their families with admissions, discharge planning, transfers and adjustments to living in a nursing home. As problem-solvers, they strive to identify social and emotional needs of the residents and arrange for their provisions, both internally and externally.

Our social workers prepare a psychosocial assessment on each resident and participate in the care planning conference with other treatment staff.

Admissions Coordinator

The admissions coordinator is a certified social worker who assists residents and family members with the admission process into Wissota Health & Regional Vent Center.

• coordinates all outside referrals from hospitals, physicians, other nursing homes, community based programs and family members
• completes a thorough assessment of each referral prior to admission. This assessment helps to assure the resident and family members that Wissota Health & Regional Vent Center is able and capable of providing all required and necessary care.
