Following is a brief benefit information summary that will help you determine the cost involved as well as which benefits are available to you according to the hours that you work. Your benefit date of coverage will be on the 1 st day of the month following 90 days of employment or after a life-changing event.

Health Insurance
Coverage is through Benefit Plan Administrators
The benefit year runs June 1- May 31.

  • Available to non-casual employees who work 30 hours per week/60 hours per pay period or more.

Dental Insurance
Coverage is through Midwest Security
The benefit year runs July 1 – June 30.

  • Available to non-casual employees who work 20 hours per week/40 hours per pay period or more.

Vision Insurance
Coverage is through VSP
The benefit year runs November 1 – October 31

  • Available to non-casual employees who work 20 hours per week/40 hours per pay period or more.

Life Insurance
Coverage is through Jefferson Pilot Financial

  • A life insurance policy is available to all non-casual employees who work 20 hours per week/40 hours per pay period or more.
  • The employer pays this premium.
  • Allows you to save income tax on the amount of money you have deducted from payroll that is set aside for reimbursement of childcare and/or out of pocket medical costs.


401K Plan

  • All employees who complete 500 hours of service within their first six months of employment can participate.
  • Contributions to the plan are based solely on your decision and are made evenly throughout the year through payroll deductions on a pre-tax basis.


Child Care

  • All full time, part-time and casual employees are eligible immediate after employment for our child are benefit.
  • Based on the number of hours you work, we will pay your child care provider a certain percentage up to a maximum of 20% per child, directly for the services they perform.


Health Club Membership

  • All full time and part time employees are eligible immediately for health club member benefits.
  • The company will pay you first three months membership fees (up to $29.99per month) as well as any enrollment fee up to $80.00 After that, you are responsible for paying the monthly fee. At the end of the calendar year, as long as you maintain an average of ten or more visits per month consistently during the year, and you are still an active employee, the company will reimburse you for the entire year, up to the maximum of $29.99 per month.
  • Your health club must submit a utilization repot to the facility upon request to verify the number of visits during the year.


Voluntary Insurance Products
Benefits are through AFLAC/Allstate
The benefit year run October 1 – September 30

  • A variety of products are available and can be set up as a payroll deduction, such as short-term disability, life insurance, etc.
  • No minimum hours are required to be eligible and products can be purchased at any time.
  • The AFLAC agent will contact you directly once you have reached your eligibility date.


Flexible Spending Account
The benefit year runs November 1 – October 31

  • All full time or part time employees who work an average of 15 hours or more per week are eligible to participate.
